Please complete this form as the first step in requesting to be matched with a provider at Lyn-Lake Psychotherapy & Wellness. Once we receive your submission we will try to match you with an available provider as soon as possible. This generally takes 2-5 business days. Once you are matched, the provider will contact you directly to discuss goodness of fit and scheduling. Please note we do not provide same-day or crisis appointments. Our administrative team cannot schedule therapy and wellness appointments. You will hear directly from your provider to initiate scheduling.

    COVID-19 UPDATE: PLEASE NOTE: Effective immediately, ALL IN-PERSON APPOINTMENTS ARE BEING SUSPENDED and we are ONLY OFFERING TELEHEALTH. In other words, all existing and new clients are meeting with their providers via telehealth, and there are NO onsite clinic appointments at this time. Our Wednesday Drop-In Clinic is also closed and we are not holding groups nor trainings. WE ARE ACCEPTING NEW CLIENTS and have providers with CURRENT OPENINGS. All our providers ARE OFFERING TELEHEALTH (i.e., video conferencing) sessions to new and existing clients, to prioritize your health and ensure that your services go uninterrupted during this unprecedented time.
