Policies and Procedures

Thank you for you interest in Lyn-Lake Psychotherapy & Wellness. Please review the following policies and procedures regarding our services, appointment cancellations, privacy policy, insurance, and more.

Policies and Procedures



Please alert us as soon as you know you will be missing an appointment. Most insurance plans do not cover cancelled appointments. If you cancel with fewer than 24 hours advance notice and we are not able to schedule another client for your time, you will be billed $80 for a therapy session, or $160 for a medication management session. But, if another client cannot make use of the time we have set aside for you, we will use it to review your session notes and course of treatment.

Missed therapy appointments are billed an $80 session fee. Missed medication management appointments are billed a $160 session fee. You may expect your provider to use that time to review your session notes and course of treatment.


Clients pay the session fee for the scheduled time no matter what time they arrive. Appointments cannot be extended past their scheduled hour.

Therapist Emergency or Scheduling Error

It is possible that your psychotherapist would have to cancel for either personal or professional emergencies. There is also the possibility of scheduling errors. On those rare occasions when your therapist must miss an appointment, Lyn-Lake Psychotherapy & Wellness may offer compensation for your time or transportation expenses.


Any testing, diagnoses, treatment or referrals result in what the law calls “protected health information” (PHI). Lyn-Lake Psychotherapy & Wellness uses this information to develop your course of treatment. We also may share PHI to arrange payment from your insurance company.

Clients may request in writing that Lyn-Lake Psychotherapy & Wellness refrain from using or sharing some information. Requests must specify the information clients want restricted and whether it is for treatment, payment or administration. Even though Lyn-Lake Psychotherapy & Wellness is not required to accept any limitations, we will do what we can to respect client wishes.

Clients may revoke at any time their agreement to use or share information. While revocation will take effect as soon as we receive a written request, it cannot apply to any information that already has been used or shared.
